The church dates from 1668, according to an engraving on a stone, and is located outside the walls and was one of the most important points of Tessennano spiritual life. It has a rectangular shape and the apse has frescoes depicting the Annunciation, the Guardian Angels and God, and the story of San Liberato with his trial and martyrdom. On the altar is a painting donated by Armando Lisini in 1937 representing San Liberato. In its vicinity you can still see the ruins of the house of a hermit.

The church is open only on the occasion of the feast of May 11.
